About the Role
We’re looking for an organised and proactive Care Coordinator to support the delivery of high-quality, person-centred care while helping to grow and develop our services. This is a fast-paced role requiring strong communication, rota management, and the ability to build relationships with clients, staff, and healthcare professionals.
What You’ll Be Doing
- Coordinate care packages from referral through to delivery
- Manage and optimise staff rotas (holidays, absence, coverage)
- Recruit, onboard, and support care staff
- Liaise with clients, carers, and professionals (GPs, Commissioners, Nurses)
- Maintain high standards of care, compliance, and service quality
- Conduct staff supervisions and support performance management
- Support business growth through client relationship management
- Participate in an on-call rota
What We’re Looking For
We are looking for compassionate, reliable, and dedicated individuals who want to make a difference. To be successful in this role, you should have
- Experience in a domiciliary care coordination role (preferred)
- Strong organisational and problem-solving skills
- Confident communicator with a proactive approach
- Experience with rostering systems (e.g. Access Planner desirable)
- Able to manage multiple priorities in a fast-paced environment
- Full UK driving licence (preferred)
